CDCP in Cookstown
The Canadian Dental Care Plan helps eligible Canadian residents who do not have access to private dental insurance with the cost of covered oral-health services.
Coverage is not the same as free dental care. Your income, the service being provided, CDCP fees and any required preauthorization can all affect what you pay.

Who may qualify
Eligibility is determined by the Government of Canada and must be renewed each benefit year.
You do not have access to private dental insurance or coverage through employment, a family member, a pension or a purchased plan.
You and your spouse or common-law partner, if applicable, filed Canadian tax returns for the previous year.
Your adjusted family net income is less than $90,000 for the applicable tax year.
You are a Canadian resident for tax purposes when your eligibility is assessed.

Why might there still be a balance?
You may need to pay your co-payment, any amount above the CDCP-established fee, and the full cost of services that are not covered. Ask for an estimate before treatment.
Covered care and exclusions
The CDCP covers a range of oral-health services, but frequency limits, clinical criteria and preauthorization rules may apply.
Examples
Some services—including certain crowns, dentures and sedation—may need preauthorization before treatment.
Current exclusions
Orthodontic services are also not currently available through the CDCP. Coverage rules can change, so review the current guide.

Review all four requirements on Canada.ca, including the private-insurance definition.
Have your Social Insurance Number, date of birth, address and tax information available.
Apply online through My Service Canada Account or call Service Canada for application help.
Confirm that your coverage is active and note the effective date before arranging covered care.

Not always. Your adjusted family net income may result in a co-payment, and you may also be responsible for charges above CDCP-established fees or for services the plan does not cover.
Service Canada determines eligibility and enrolment, while Sun Life administers dental benefits and claims for the CDCP.
Yes. You can discuss non-covered treatment with your dental provider and request a written estimate. You would be responsible for the cost of services that are outside your CDCP coverage.
